Is it always advisable to find out who the head of graduate recruitment is then address the cover letter to them personally? Or, if the firm hasn't advised on this just refer to the graduate recruitment team?

You mention this is for a pre-employment check - you’ll need to flag this discrepancy to the people doing the reference checks and they may flag it to the recruitment team who will make a decision on whether it is an issue or not. The multiple discrepancies could make this an issue.

If the reference company or recruitment team ask you about this, be truthful as to why both discrepancies happened - there isn’t much else you can do at this stage now.
