Unless explicitly asked to do this, then no. Waste of word count, to add the firms’ and yours.
If you’re asked to upload a word doc as a cover letter, then yes you should. One of the things that may be assessed is if you can format a letter, so it’s better to play it safe.
I typically didn't include addresses but I would include the date (aligned to the right) and then "Dear Graduate Recruitment,", followed by "Yours faithfully," at the closing.
