1. Yes, should be fine. I think there is a drop-down option within the work experience section, where you state whether the role was paid/unpaid, etc.
2. I think it depends on the firm, if they are known to be traditional with cover letter format requirements, I would advise doing the latter...

Typical M&A case study, be concise, to the point and watch the time. I would brush up on typical issues in an M&A deal for instance, litigation, balance sheets, competition, diversifying products...

I think it really depends on the firm, what I have noticed from my own experience is that post 5pm, I usually receive rejections and pre 5pm, I usually get offers.
