How would one write an effective and punchy executive summary for a written exercise portion of an AC?

What should it look like? What must be included? Should it be in bullet points or prose?

Imagine that you have the attention of your audience for around a minute. What messaging would you want to convey? That's your executive summary. It should be in prose. Bullet points can be used to a limited degree where appropriate.

Can it contain headings? For example: issues, recommendations, next steps.

Yes, you can use headings - but if the document is less than a page, they may not be needed where the writing in itself is fairly short.
